Case Management Services Easter Seals Florida offers two case management services: the Family Resources Project and Senior Concierge. Easter Seals Florida recently launched the Family Resources Project, a single point-of-entry, comprehensive individual- and family-centered case management program to assist individuals with disabilities and special needs. Needs assesments have been conducted in Lee and Collier Counties over the last two years, which informed the development of this program. The Family Resources Project helps break down the barriers to services along a continuum to ensure the entire individual is served, resulting in stress alleviation and improved access to all that is offered in Southwest Florida (and beyond).
