The completed camp application must be sent with a $50.00 nonrefundable fee. Applications sent without the fee will not be processed unless a funding source (i.e. clubs, community organizations and/or churches) has agreed to pay the fee. In this case, a “Letter of Intent to Fund” must be received and put on file. A copy of this form is enclosed and must be submitted to the agency or organization that will do the funding and then faxed or mailed to the camp to put on file. You will not receive confirmation until that form is on file. You may make as many copies of the form that you will need.

One half of the fee is due no later than June 1st, and the balance is due by the first day of the camp session unless other arrangements have been made and the “Letter of Intent to Fund” is on file.

If you are on the Maryland Autism Waiver Program you must confirm with your coordinator the number of hours you will need for your session and have a copy of the Plan of Care, with Easter Seals as a provider and the hours sent to the Camp. If a participant attends and do not have the hours available, the parent/caregiver will be responsible for the balance due.

How to Register:
Complete the (1) registration form, the (2) participant information form, the (3) Letter of Intent, and mail with the $50 registration fee to Camp Fairlee, 22242 Bay Shore Road, Chestertown, MD 21620. Or you may register online at www.de.easterseals.com/fairlee.
Once steps 1, 2, and 3 have been completed, a confirmation letter and the health form will be sent to you. If you are a new participant, you will not receive anything until you have completed the precamp interview.

Refunds:
Participants will receive refund of the fee received if they cancel no later than one week prior to registered session. The registration fee of $50 is non-refundable. Participants who choose to withdraw while program is in session will not receive any refund. Exception will only be made in case of family or medical emergencies. In such cases, a pro-rated refund will be applied. All refunds must be approved by the Camp Director.

Health Services:
At least one nurse will be available on site at all times. A physician is on call for consultation when appropriate. Health services are limited to procedures which can safely and effectively be delivered at camp. A completed health form must be received prior to participant’s arrival to camp. All forms must bear a signature of licensed physician who prescribed medication(s) and/or have reviewed the form. In addition, this form must be current, that is physician’s review must be within 12 months of camp attendance.

Participants should be ambulatory or able to transfer to a passenger van. All participants must be able to participate in a 3 to 1 camper to counselor ratio. Camp Fairlee reserves the right to send a participant home due to behavioral or medical issues, at the camper’s expense. This decision will be made by the Director.

In order to register for our travel trips a completed registration form, participant information form, Letter of Intent (if an agency or organization is paying) and a $400 deposit must be received by our office. All applications must be received at least 3 weeks prior to the trips departure date, so that arrangements can be made in a timely manner. If an organization, club, or agency will be funding the trip a “Letter of Intent for Funding” must be received and on file in order to complete the registration process. A form is enclosed or the form can be faxed to the funder by the Camp. You must call and make this request.

Once the (1) deposit, (2)registration form, (3) the participant information and (4) the letter of intent have been received, you will receive confirmation of acceptance and a health form that will need to be completed prior to the trip and sent in to Camp.
If you are a new participant, you will not receive anything until you have completed the precamp interview.

Refund Policy:
All cancellations must be made in a timely matter. (Someone may be on the waiting list.) There will be no refunds for late cancellations. Any refund exceptions will be made by the Administration.
Participants may be refused entrance, with no refund, if they arrive without all prescribed medications or refuse to board the vans, plane, or ship.
